So, in reading bits of this article it seems fairly obvious to me that once the crew takes off their helmets, this is how they are originally infected.
Shaw sees "something" moving around on the ceiling and some crew members are hit by falling drops of what they assume is water.
If you look at the closeup of the ampule David is staring at, there are definitely rivulets of water on the sides.
This initial foray doesn't seem to do any harm, and the crew returns to the ship, with shaw carrying the space jockey helmet, but seemingly okay. I also think David will have taken the ampule he is examining, and some of the clear plasma material.
The Engineers first response is to send a sandstorm which hits Shaw and bangs her up.
The crew manages to study the helmet, David studies the creatures in the ampule, and Shaw and Holloway have sex.
Later, David sees the object in his eye, other things start to get weird, and they decide to return to the temple to learn more. This is where the infections really start to go haywire, some people die, including Holloway, and the crew makes it back to the ship and decide to leave.
Shaw is analyzed and found to be pregnant, and fights away from the medical crew to deliver the worm in the med pod herself.
Everything goes downhill from there.
